Qventus launched the Care Gap and Coding Automation Suite in February as a system of action embedded in existing EHR workflows to identify missed diagnoses, orchestrate timely clinical interventions, and complete documentation in real time. The suite shifts clinical interventions left, surfacing conditions the moment the patient is admitted and taking action, rather than a delayed retrospective audit after discharge.
The first automation in the suite, Malnutrition Care Automation, uses chart mining to identify malnourished patients, pre-populates the nutrition consult order, surfaces key insights to dietitians to aid in assessment, and prompts diagnosis documentation for MCC/CC capture before the coding window closes.

Qventus launched the second solution in the Care Gap and Coding Suite to identify patients at risk for pressure injuries. Embedded in existing EHR workflows, it continuously analyzes patient data to identify patients at elevated risk of having or developing a pressure injury and orchestrates the next clinical action, including initiating prevention order sets, recommending support surfaces, prompting timely skin assessments, and confirming that repositioning interventions are completed. Documentation follows the same path, capturing skin assessment findings, present-on-admission status, prevention activities, and pressure injury staging as care is delivered rather than after discharge.
Present-on-admission accuracy carries particular weight for pressure injuries. An injury documented during the admission window is recorded as present on admission; the same injury captured later is counted as hospital-acquired, which affects performance under the CMS Hospital-Acquired Condition Reduction Program.
